Message from the CEO
Country SA PHN (CSAPHN) has recently sought to re-commission a wide range of allied health and chronic disease activities, currently provided across mostly northern parts of country SA. The re-commissioning was directed by the Commonwealth as part of its strategy to enhance service delivery to the community, in chronic disease support for rural people and to better integrate services and improve local community sustainability and resilience in health care.

Tenders were let for services across the West, North, Mid North and Yorke Peninsula and through the Mallee, Kangaroo Island and down to Kingston and Robe. The Tenders for services were specifically targeted to local government areas of fewer than 5000 persons and for services to be delivered inside the communities and well connected to general practice and local allied health and community health providers. Health Care Homes trial - Update

The Health Care Home (HCH) trial officially started on 1 December 2017 for the 16 registered sites in the Country SA PHN region.  As an assistive resource to the Australian Department of Health, our team of enthusiastic Practice Facilitators have been working closely with HCH General Practice and Aboriginal Community Controlled Health Service staff during the last couple of months, in readiness for this start date.

With our sites varying from small to large, regional to remote, our team are tailoring assistance to fit the need of each site.  The response to-date has been very positive and we look forward to providing a window into activities in the coming year.

Early in 2018, Country SA PHN will be working towards making available a range of tools and training to support our HCHs and their Medical neighbourhoods in the trial including access to a Shared Care Platform.
