Message from the Chair

The Country SA PHN is looking at setting up a new engagement platform as a means of collating feedback from our stakeholders and creating a space for secure chat. 

The platform will be known as Health Connections Community and is currently hosting information and opportunities for discussion for the Yorke Peninsula Pain project and several other CSAPHN initiatives. You can have a look at the website here and contact the PHN for login details.
We will be looking to use this as a platform for health practitioners to bring issues to the attention of the PHN and function as one aspect of our clinical council. Read more...
Health Care Homes Patient Information Forum
Country SA PHN's Practice Support team met with over 50 patients from the Angaston Medical Centre on Tuesday evening as part of the medical centre’s Health Care Homes Patient Information Forum.
Patients were informed about the Health Care Homes Trial, which develops shared care plans for patients with chronic and complex conditions. The large turnout of patients were provided with information on the role of CSAPHN, Chronic Disease Management and care services provided at the Angaston Medical Centre.
Well done to all involved and a special mention to Janine Gladigau, Medical Practice Assistant at Angaston Medical Centre.

PenCS Suite (CAT Plus) training

PenCS Suite (CAT Plus) training was conducted with Dr Jerome Connolly and Jodie Boniner Practice Manager of the McRitchie Crescent Surgery together with Country SA PHN staff member Debbie Stratford discussing the new Practice Health Profile (PHP) and the use of CAT Plus which is available to all general practices with the Country SA PHN Region.

Rural Health Workforce Scholarships Recipient - Sharon Bilney

As part of the RDWA Rural Health Workforce Scholarship Program, Sharon received a $3,500 bursary to assist her in completing the final units of study in her Master of Public Health (Public Health) from Flinders University.

Sharon is now the first Aboriginal woman in Port Lincoln to have completed a Master’s Degree in a health related field and hopes to use this opportunity to inspire other young Aboriginal women in the community to pursue further education.

Regional Disability and Wellbeing forum
This month the Yorke and North team supported the Regional Disability and Wellbeing forum held at the Northern Festival Centre in Port Pirie.The forum, which attracted over 60 attendees, brought together a broad range of key stakeholders from across the disability sector, local government, health and services ecosystem. 
The forum explored, shared and heard insights from a varied range of guest speakers who investigated strategies of collaboration. Topics of discussion included; appropriate recruiting, training for skills, inclusive Healthy Communities and building capacity through the NDIS. CSAPHN Yorke and North Regional Manager, Emma Young, presented on Health and Wellbeing in communities.

Tristar Group – Mount Gambier

A successful Health Care Home (HCH) patient enrolment session was held at the Tristar Mount Gambier Practice on Tuesday 28 August 2018. Pictured are Dr Mathew Oemen and Ms Wendy Norman, Senior Practice Nurse, who have now registered their first patients in the HCH trial.

The Country SA PHN Practice Support team are busy assisting the participating Health Care Home practices build on the integrated and patient-centred care model to become high performing general practices.

Australian Scout Jamboree - Help Wanted

How would you like to work at an event where children are having heaps of fun, without electronic devices!  I am looking for voluntary assistance to help with a medical centre at the Australian Scout Jamboree to be held at The Bend, Tailem Bend in early January 2019.

SA Coroner's Recommendations

The State Coroner made a recommendation that the general practitioner community are reminded that indwelling catheters double the risk of delirium and promote bed-centred care and that catheter associated urinary tract infections do not require antibiotic treatment when there is no evidence of a more generalised infection.

Move It Aus - Better Ageing Grant Program

The Better Ageing Grant Program aims to increase levels of physical activity of Australians aged 65+, particularly those least active, to improve their overall health and wellbeing. The new $22.9 million grants program is now available to national sporting organisations and physical activity providers.

SA Health Interim Medication Administration Chart (IMAC)
SA Health has developed a standard interim medication administration chart (IMAC) for use when patients transfer from hospital to a residential care facility.
The standard SA Health IMAC will replace the variety of individual hospital versions that are currently used.

Under work health and safety laws, everyone is responsible for health and safety in the workplace. The legislation defines health to include both physical and psychological health. Download the flyer or watch the video.

Minlaton Medical Centre
Are currently looking for another GP to join our busy practice in Minlaton on the beautiful Yorke Peninsula about 200kms from Adelaide CBD.

The practice is one of 5 practices which are owned by Adelaide Unicare which is a controlled entity of the University of Adelaide. The practice currently comprises 3 GPs who are well supported by our highly motivated team of practice nurses and administrative staff.
